Application Process
|
|
1. If you are over the age of 55, or you have a permanent disability, you may call our offices for an application to be faxed or emailed or download the application from this web site. Please note we no longer mail the applications. Before you call, please note that because of the number of applications we receive, we are not able to provide emergency care, and our wait list is about three years. A list of counties that are CLOSED for applications as of February 2006 is below.
2. Once you receive the application, please fill it out and send it back to us.
3. Applications are processed in one big batch at the end of the month. Because of the number of applications we receive, we only notify people by mail if they DO NOT qualify for the program.
4. Unless you have heard from us by mail that your application has been denied, your name will be added to our wait list. When your name comes to the top of the list, you will be notified by mail, and a DDS caseworker will interview you by telephone.
5. When you are matched up with a dentist, you go to their office for treatment. You will be responsible for getting to your appointments on time and treating the office staff with respect. Missed or late appointments are grounds for immediate termination, no questions asked. Our volunteer dentist will provide us with your treatment plan, and we coordinate specialist referrals and necessary labwork in order to complete that treatment.
6. Once the recommended treatment plan is complete, your case is completed and you cannot receive care through DDS again, so that we may continue helping others who have been waiting for care.
